We collect personal and health information to provide high-quality occupational health and wellbeing services, comply with legal requirements, and improve our service delivery.

  • Administrative information: appointment records, invoices, and correspondence

Where possible, we collect this information directly from you. In some cases, with your consent, we may collect information from your employer, GP, or another healthcare provider.

  • Conducting health checks and assessments

  • Preparing health summaries or fitness-for-work reports

  • Contacting you about appointments, results, or follow-ups

  • Referring you to another provider (with your consent)

  • Meeting our legal, regulatory, and professional obligations

  • Quality assurance and service improvement (using de-identified data)

We will not use your information for any purpose that you would not reasonably expect.


4. Sharing Your Information

We respect your confidentiality. Your personal health information will only be shared when:

  • You give explicit written consent for us to share results with your employer or another provider

  • We are legally required to disclose information (e.g. notifiable diseases or court orders)

  • It is necessary to prevent a serious threat to your health or safety, or that of another person

We may provide de-identified or aggregated data to employers for reporting trends or compliance purposes, but individuals will not be identifiable.


5. Storage and Security

We take all reasonable steps to keep your information safe from loss, misuse, and unauthorised access.
This includes:

  • Secure electronic storage systems with password protection and restricted access

  • Encrypted communication where appropriate

  • Regular audits of our data management systems

Health Check NZ complies with the Health Information Security Framework standards for data protection.


6. Access and Correction

You have the right to:

  • Access the personal information we hold about you; and

  • Request corrections to ensure it is accurate, complete, and up to date.

To make a request, contact us using the details below. We will respond within 20 working days as required by law.


7. Retention of Information

We retain health records in accordance with New Zealand health record-keeping requirements — generally for 10 yearsf rom the date of the last contact, unless otherwise required by law.

After this period, records are securely destroyed or permanently anonymised.


8. Website and Digital Information

When you visit our website:

  • We may collect general usage data such as IP address, browser type, and pages visited to improve user experience.

  • Cookies may be used for website analytics and functionality; you can disable them in your browser settings.
    We do not collect or store identifiable personal health information through our website.
